To make games easier to run, streamlining administrative tasks and establishing player consent, player forms can be useful.  Here are some standard forms I've used at my games.

Game Waiver - A Waiver of Liability form.  While not ironclad legally, it sets a basic understanding of the risks a player runs at a private field and records their consent to the risk involved in participation.
